Aside from accessing the I/O address space to read input status or read/modify outputs, CartIO32.dll does not do anything else. This means that, if before the card is to receive commands to initialize correctly, it must be configured (by hard and/or soft) so that as soon as it is switched on and then when the workstation is booted, it be operational.
For example, on the Advantech PCI-1751 board, which has 6 'groups' that can be used as input or output, jumpers must be set if you want to force the groups to operate in 'output' mode. Otherwise, the state is undefined and the operation random.
